Carers Services

We offer a range of services to aid those who care for others, feel free to contact the surgery for more information and advice or visit the NHS website.

Cervical Screening

Cervical screening is a method of preventing cervical cancer by detecting abnormal cells in the cervix. Women between the ages of 25 and 49 are invited to be screened once every three years, and women aged between 50 and 64 are invited to be screened once every 5 years. If you have have a history of abnormal smear results you will be invited for screening more regularly.

If you are due a cervical smear the Surgery and Local Health Authority will contact you inviting you to book an appointment to be screened, you can make an appointment at one of the following clinics in Hammersmith and Fulham - "Smear Screening Centres", or book an appointment with one of the Nurses at Dr. Jefferies and Partners.

Make sure we have your most recent Smear Test Results by filling in a simple form.

Child Immunisation Clinic

These are held every Tuesday afternoon between 1.30pm - 2.45pm.

We strongly recommend that all children are fully immunised against Diphtheria, Tetanus, Polio, Whooping Cough, Measles, Mumps, German Measles (rubella), Meningitis C and haemophilus. All these immunisations are available at the Practice.

Sick children should not be brought to this clinic, but will be seen during normal surgery hours.

Chronic Disease Management

Long term illness clinics are available for the following diseases:

• Diabetes
  
• Bronchitis
  
• Asthma
  
• Chronic Heart Disease
  
• Thyroid problems
  
• High Blood Pressure

These clinics are run by the Nurse, the Doctor and a Health Care Assistant. Patients with any of the above conditions will be required to have a review at least once a year to ensure they are well controlled and that they are receiving the most adequate therapy for their condition. The patient will see the Nurse or Health Care Assistant for a general review which includes physical examination, some tests (blood tests, lung function, etc) and education.

Depending on how well controlled you are sometimes the Nurse will ask you to see the Doctor to review the results and management. Some of these reviews take longer that others, so when you book your appointment please mention which review you need so we have enough time to carry out a thorough check.

Specialist nursing services work closely with the Practice, providing care in the home.

Family Planning

A contraceptive service is available from the Practice Nurse and the Doctors. Please make an appointment during normal surgery hours.

Doctors can provide you contraception pills, injections and patches on prescription. We do implants insertion. In addition to this, we give advice on long term contraceptive methods for example: IUD, Mirena, Sterlisation etc.

Repeat pill prescriptions are available from our nurses.

New Family Planning Clinic at Munster Road

The family planning clinic will run most Tuesdays from 4.00pm - 6.00pm from the beginning of August 2012.

Family planning clinics can be used by women, men and young people, including those who are under 16 years of age.
You can refer yourself to a family planning clinic, or you can be referred by your GP, or another health worker.

The clinic will be run by:

Family planning clinics provide a range of services including:

• Fitting and checking of coils (intrauterine devices, or IUDs)
• Confidential advice about contraception
• The combined oral contraceptive pill 
• The progestogen-only pill 
• Progestogen injections 
• Free emergency contraception
• Confidential advice about STIs
• Cervical screening 
• Unplanned pregnancy advice 
• Pre-conception advice and fertility awareness information
• Fitting and checking of caps, diaphragms, and coils (intrauterine devices, or IUDs).
• Implants insertion

There are a number of services that are run from these premises that are offered on a Private basis. They are not part of the NHS, so fees are usually payable.

Hospital Referrals

Patient Choice
A referral to hospital can be a daunting experience, fortunately, since April 2008 - there has been the introduction of Free Choice

Free Choice means that if you're referred to see a specialist, you can now choose to be treated at any hospital in England that meets NHS standards. This includes NHS hospitals and independent hospitals.

Free Choice is simple and effective so if you need a hospital referral ask your GP about your right to choose.

Choose and Book
Choose and Book is a service that lets you choose your hospital or clinic and book your first appointment. When you and your GP agree that you need an appointment, you can choose which hospital or clinic you go to. You will also be able to choose the date and time of your appointment.

Influenza Vaccination

Flu vaccination is available for all our vulnerable patients. This includes patients who are over 65 years, together with diabetics, asthmatics and patients with heart problems. We recommend most strongly that if you fall into one of these groups that you have a flu vaccination each year.

The flu vaccination does not give you the flu.

Methotrexate Clinic

Some patients need to take Methotrexate for certain conditions such as arthritis or psoriasis.

This drug suppresses your immune system which keeps your disease and symptoms under control. Although, this is a highly toxic drug and therefore needs to be monitored closely. In the Methotrexate Clinic, which is led by a Practice Nurse and supervised by a senior GP, we take routine blood tests every six weeks to make sure your body is responding well to the drug and that you do not experience any side effect or complications.

These clinics are run every Thursday morning between 8.00am - 10.00am.

You cannot get a prescription for Methotrexate unless you attend this clinic.

If your Doctor has prescribed you Methotrexate and you are not being monitored anywhere else please make an appointment for the Methotrexate clinic.

Midwives

Antenatal and postnatal care is shared between a hospital specialist, your GP Practice and the local Midwife.

Minor Surgery

Various procedures such as, excisions, injections in to the joints, implants and abscess drainage.

Patient must be seen by a doctor prior to booking an appointment in the minor surgery clinic.

Musculoskeletal Clinic

The Musculoskeletal Clinic is concerned with pain in the movement system. This can have many causes and often it is not possible to narrow things down to one cause with one effective treatment.

Therefore we explore many areas which could contribute to the experience of pain. This ranges from the body’s response to stress, to the way posture, sleep, nutrition, ageing and occupation influence the way we move and react to signals from our body.

There are many ways to relieve pain and improve function. This clinic is dedicated to help you in finding ways of improving your health.

There are several therapies we offer which can be helpful in restoring function and alleviating distress:

• Acupuncture. 
  
• Manual Medicine approaches like Myofascial release and Manipulations. 
  
• Exercises. 
  
• Joint injections with steroid or local anaesthetic. 
  
• Information about the body and its response to internal and external events.

This clinic is ran every Thursday afternoon on an appointment basis by Dr Foell.

Non NHS Services

The Practice is able to provide a range of services not covered by the NHS. These include certain travel vaccinations, medical insurance and legal reports, pre-employment medicals and HGV driving medicals. An appointment will need to be made and a standard fee will be charged. Full details are available from reception.

Nursing Team

The nursing team at the Surgery consists of 2 - 3 Practice Nurses who are available by appointment for a full range of nursing services such as:-

• Immunisations, including the flu vaccination from October each year & the pneumococcal vaccination (check with the nurse to see if you are eligible for these vaccinations on the NHS).
  
• Wound dressings
  
• Blood Pressure checks
  
• Health Promotion and long term illness clinics
  
• Cervical smears; remember that it is important for all women under the age of 65 years who have not had a hysterectomy, to have regular smears every three years
  
• Travel vaccinations; please leave plenty of time before travelling, as some vaccinations need to be given up to 4-6 weeks in advance of travelling. A fee will be charged for some vaccinations that are not covered on the NHS.

Phlebotomy

If you require a blood test, this will be carried out by appointment with the Health Care Assistant or the Practice Nurse.

It is very important that you are aware whether or not this is a fasting blood test, in which case nothing can be eaten or drunk for 12 hours before the blood being taken. Click here if you are having a Fasting Blood Test.

Pneumococcal Vaccination

The pneumococcal vaccination is also available for the over 65’s and those who are at risk i.e. Diabetics, Asthmatics, COPD patients etc. Please check with the Nurse to see whether you are eligible for this vaccination on the NHS.

Travel Vaccination Clinic

• Our Nurses are able to provide comprehensive travel advice. This includes immunisation, malaria prophylaxis, information and general advice on health and safety while travelling abroad.
• Some vaccines are not stocked but can be ordered or obtained from the pharmacy. If you have a query about these please contact the Surgery.
• You must leave sufficient time for your vaccination programme to be completed, before you travel. This should be a minimum of four weeks before departure, but 6-8 weeks is preferable.
• The Practice is able to offer a range of NHS and private vaccinations. Private vaccinations, such as Yellow Fever, Japanese Encephalitis and Meningitis ACWY are charged for, as they are not available on the NHS.
• Our Nurses will be able to advise you on what vaccines you require for your particular destination.

Well Women Services

Our Nurses provide a number of services aimed at the specific needs of women. These include the following:

• Cervical smears
  
• Health Advice
  
• Breast check advice
  
• Menopause advice
  
• Pre-menstrual syndrome
  
• Screening for certain infections
  
• Pre conception advice

Warfarin Clinic

If you are taking Anticoagulants (most commonly Warfarin, sometimes Sinthrome) for a short period of time or as a lifelong treatment, you should be attending an Anticoagulation clinic. This group of drugs makes blood to take longer to clot and can make you prone to bleed for which is necessary to monitor your blood closely.

In this clinic, which is led by a nurse/healthcare assistant and supervised by a senior GP, we take a blood sample from your finger and perform a test that measures how long your blood takes to clot and adjust your medication dose in order to keep your therapy safe and under control. It is extremely important that patients on this treatment attend regularly to reduce the risk of complications.

If your Doctor has prescribed you Warfarin, Sinthrome or any other anticoagulant (please note that Aspirin is not an anticoagulant) and you are not being monitored anywhere else please make an appointment for the Anticoagulation Clinic.

These clinics are run every Monday and Wednesday from 1.30pm - 3.30 pm, and Friday from 10.00am - 12.00pm.
